Needs Assessment: Dear Ranch or Farm Family

July 17, 2006

Dear Ranch or Farm Family,

Are you or is someone you know ranching or farming with a continuing medical or physical problem? There is help available for ranch and farm families who want to stay in agriculture even with physical challenges or disabilities. AgrAbility provides information, education, and services. Colorado State University Cooperative Extension and Easter Seals Colorado work together to help them continue to do what they enjoy - - farm or ranch.

To get the word out to farm and ranch families with physical or medical challenges or disabilities, we need your help. We can either send you information for you to pass on to these families, or we can send information directly to them. All you need to do is check off the kinds of information that are needed and let me know.

We have/I know someone who has the following disability or disabilities:

I want to refer someone for services. Below is his/her name and telephone number or mailing address

I want to learn more about:
Making a home or worksite more accessible
Receiving on-site assistance to deal with the following problem related to a physical challenge or disability:
AgrAbility workshops for farm and ranch families with physical challenges or disabilities
AgrAbility workshops for professionals who work with farmers/ranchers with disabilities
Talking via phone with another farmer or rancher with the following disability:

I want to volunteer:

With a monetary donation of: $

Please send AgrAbility information to:

Name,address,city,state,zip,phone number, e-mail address, please.

Thanks very much for helping us get the word out about AgrAbility to farm and ranch families. If you have any suggestions about how we can do a better job of reaching these folks, won't you please give me a call at 970-491-5648 or drop me a line? I'd enjoy hearing from you.

Very sincerely,
